In the interest of providing the best quality of care and to allow you the most time possible with the physician, Mobility Medicine is a Fee for Service Clinic.  Health insurance companies will likely cover some or all of the cost and can be submitted following the visit for reimbursement.  We accept all major credit cards.  No personal checks please.  We also accept Flexible and Healthcare Spending Accounts (FSA & HSA).

Insurance:

We do not participate with Insurance Companies at this time.  We will work with you to get reimbursement from your Insurance Carrier if you would like.  This usually entails filling out a short one page form and mailing it, along with our itemized reciept (Super Bill) to your Insurance Company.

Medicare Patients: We are a non-participating member of Medicare which means after your visit our office will submit a claim on your behalf for Medicare to send a reimbursement check to you.  There is no guarantee how much or what will be covered, so we also encourage you to discuss what will be covered with Medicare and your secondary provider if you have one.  Patients with Medicare Advantage plans will submit claims themselves.

Patients have received reimbursement from the following insurances:

  • Blue Cross and Blue Shield of Michigan

  • Medicare

  • Priority Health

  • Aetna

  • United Health Care

Since each Insurance Plan is different there is no guarantee that our services will be covered, so we encourage you to check with your insurance provider to make sure what they will cover.  Medical Billing codes we usually use include:

  • 99213 or 99214 (Follow Up Visit)

  • 99203 or 99204 (New Patient Visit)

  • 98925-98929 (Osteopathic Manipulative Treatment)
